Learn how to Refer to CNS. For providers, CNS may require a referral from the patient’s primary care physician (PCP), primary care manager (PCM) or other designated primary care provider, or another referring provider to include specialists/facilities. Some health plans require a referral and in some cases prior authorization may be required.
The referring provider should indicate the reason for the referral or chief complaint and purpose of the visit. Documenting a clear reason for the referral will allow the neurologist to focus on the request and establish a productive initial visit.
Example referral reasons, but not limited to:
- Abnormal Brain Scan
- Back Pain
- Carpal Tunnel Unspecified
- Dementia
- Dizziness/Giddiness
- Gait Disorder
- Headache
- Lower Leg Pain
- Memory Loss
- Muscle Weakness
- Neck Pain
- Numbness
- Slurred Speech
- Syncope
- Stroke
- Tremors/Twitches
- Ulnar Nerve Injury
CNS accepts many national and local health plans. In addition, a designated specialist accepts most national and local referrals from worker’s compensation companies, Medpay insurance, and medical lien case referrals. Secure Insurance Provider Portal
Some secure portals include, but not limited to the following, Availity, Centene/Silver Summit, Health Net, Optum, Sierra, etc.
Secure Direct Messaging
CNS accepts EHR secure Direct Messaging. Referring providers can call the office at 702-804-1212 Option 3 and speak with the Medical Records Department for further assistance on secure direct messaging.
By Secure Fax
Please fax the completed 4 Step Referral Form along with progress notes to 702-577-0985. Kindly include a copy of the patient’s ID and insurance card(s) front and back, along with any pertinent diagnostic testing (e.g. MRI Brain).
By Mail
Please mail in the completed 4 Step Referral Form along with progress notes to Clinical Neurology Specialists at 7751 W. Flamingo Rd, Suite A-100, Las Vegas, NV 89147 ATT: New Patient Scheduling. Kindly include a copy of the patient’s ID and insurance card(s) front and back, along with any pertinent diagnostic testing (e.g. MRI Brain).

To check status of a medical evaluation report, or to arrange a physician-to-physician call, please fax the request on company letterhead to 702-804-1273 Monday – Friday 7:00 AM to 4:00 PM.
