Do I Need a Referral?
CNS may require a referral from one of the following:
- Primary Care Provider (PCP)
- Primary Care Manager (PCM)
- Other Referring Providers (e.g., specialists, urgent care, or facilities)
Some insurance plans require referrals or prior authorizations before scheduling. Please verify with the patient’s health plan prior to sending the referral.
What to Include With the Referral
To ensure a productive and focused initial visit, please provide the reason for the referral, including:
- Chief complaint or symptom(s)
- Any relevant findings or suspected diagnoses
- Supporting clinical documentation
Examples of common referral reasons include (but are not limited to):
- Abnormal brain scan
- Headache or migraine
- Memory loss or dementia
- Dizziness or giddiness
- Gait disturbance
- Muscle weakness or numbness
- Neck, back, or lower leg pain
- Tremors, twitches, or slurred speech
- Stroke, TIA, or syncope
- Carpal tunnel or ulnar nerve injury

You may refer your patient to CNS using any of the secure methods below:

Please include the completed form with:
- Recent progress notes
- Patient’s photo ID and insurance card(s), front and back
- Any relevant imaging or diagnostic results (e.g., MRI Brain)
Through Secure Insurance Provider Portals
CNS accepts referrals through various insurance portals, including but not limited to:
- Availity
- Centene/Silver Summit
- Health Net
- Optum
- Sierra Health
Via Secure Direct Messaging (EHR-to-EHR)
We accept EHR referrals through secure Direct Messaging. Please contact our Medical Records Department at 702-804-1212, Option 3 for setup and assistance.
